東京生まれ。東京外国語大学スペイン科卒業後、総合雑誌『現代の眼』編集者を経て、60年代半ばから写真を撮りはじめ、同時期よりさまざまな雑誌に写真や映画に関する執筆を開始する。68～70年には多木浩二、高梨豊、岡田隆彦、森山大道とともに「思想のための挑発的資料」と銘打った写真同人誌『プロヴォーク』を刊行。70年に写真集『来たるべき言葉のために』を上梓した後、73年には映像論集『なぜ、植物図鑑か』で、それまでの自作を批判的に検証。77年に篠山紀信との共著『決闘写真論』を刊行直後、病に倒れて生死の境をさまよい、記憶の大半を失うが、以後も写真家としての活動を継続。2003年には横浜美術館で初期から2003年にいたる800点におよぶ作品群による「中平卓馬展 原点復帰－横浜」を開催し、その図録を兼ねた写真集『原点復帰－横浜』刊行。以降も新作による個展開催、また内外のグループ展にも参加。2011年には大阪Sixにて、大規模な新作展「キリカエ」を開催された。
